Medicare and Medicaid
At this time, we do not accept any Medicare or Medicaid/Medical Assistance plans, including managed care or advantage plans. While we may offer services to Medicare and Medicaid beneficiaries in the future, this requires time and resources that are currently beyond our capacity as a small practice. Please note: Medicare and Medicaid beneficiaries typically cannot pay out of pocket for services.

Out-of-Network Insurance
If I am not in-network with your insurance, you may still be able to use out-of-network benefits for therapy. This involves paying the full session fee at the time of service. Upon request, I can provide a superbill—a detailed receipt of the services provided—for you to submit to your insurance company for potential reimbursement. If approved, your insurance may reimburse a portion of the cost.*

No-Show/Late Cancellation Policy
A $65 fee applies for missed sessions or cancellations made with less than 24 hours’ notice. This fee is charged regardless of whether sessions are typically covered by insurance or paid at a reduced rate. Please note that insurance does not cover no-show or late cancellation fees.

Whenever possible, please cancel appointments with as much notice as you can. This allows you to reschedule and makes the time available for others.

*Please note: As your provider, I cannot determine how your specific insurance policy covers mental health services. Policies vary widely. Some require a flat copay, while others involve coinsurance, which is a percentage of the session cost. To verify your benefits, contact your insurance provider using the "For Members" customer service number on your card or check your benefits online. Look specifically for coverage details under "Outpatient Mental Health Care" to understand your expected costs.

Evaluation and letter of support for gender affirming (confirming) surgery adhering to WPATH standards when such letter is determined to be necessary and appropriate.

Individuals seeking gender affirming surgery should be able to do so without systemic barriers. Evaluations for a letter supporting gender confirming surgery are focused on informed consent, identifying supports as well as any potential psychosocial issues which might negatively impact recovery.

Insurance may cover the evaluation and follow up consultation to discuss the letter and recommendations.

The cost includes up to 30 minutes of professional consultation with the treating provider and time crafting and distributing the letter; letter writing services are not covered by insurance and will be billed directly to the client.

For those clients without insurance to cover the cost of the consultation, a reduced fee may be available:
$0-$165 - Established Client Evaluation + Letter
$0-$350 - New Client Evaluation + Letter

Clients may choose to continue with therapy and other services provided after the evaluation, based on therapist’s availability. Please see posted fees.

The Gender Affirmative Letter Access Project (GALAP) is a group of clinicians offering pro bono letter writing services for clients seeking gender affirming surgery. If Gretchen Murchison is unable to provide the service you are seeking, you may wish to check with GALAP for available clinicians.

Following an assessment, completion of a form (supplied by client) or letter of recommendation for accommodations or a letter of support when such letter is determined to be necessary and appropriate. Document is released to client only unless direct submission is required.

A signed Release of Information is required for any communication between therapist and outside parties.

The cost covers a 30-60 minute evaluation, 30-45 minute follow up consultation to discuss the contents of the document, and time completing and distributing the document. Assessment and follow up session may be covered by insurance.

RENTER’S LETTER OF SUPPORT FOR ESA:
Letter supports anticipated benefit to client from having an Emotional Support Animal (ESA) when such letter is determined to be necessary and appropriate. ESAs are companion animals not specifically trained to be service animals. Letter does NOT certify animals as ESA and Sunrise LCSW / therapist assumes no responsibility or liability for animal behavior.

A consultation is an assessment or service event provided by the therapist whose opinion or advice regarding evaluation and/or management of a specific problem is requested by another clinician or other appropriate source. For example, if the therapist is asked to see an individual for a pre-operative clearance or for evaluation of a specific problem. The consultation event is not an intake and is not intended to initiate an ongoing therapeutic relationship.

The cost covers the 30-60 minute evaluation, a 30-minute follow up if necessary, up to 30 minutes of professional consultation with the referring provider, and time crafting and distributing a consultation summary.

Consultation and follow up session may be covered by insurance.

Real-time telephone or video communication with a designated party at client’s request, outside of a session, with signed written Release of Information, and when determined to be necessary and appropriate. Not billable to insurance.

The therapist’s role is to provide therapeutic services so that individuals might feel better and/or improve functioning, at times as it relates to family dynamics. The therapist’s role is not intended to gather information for the courts or to make judgments related to the family.

It is our policy to have no court involvement in family court cases as that could harm the professional relationship and the ability to achieve treatment goals.

The informed consent includes a clause that clients agree not to request testimony in any future court proceedings or allow release of therapeutic intervention records.

Courts can appoint experienced, specially trained professionals to conduct independent evaluations and make recommendations to the court when needed related to a divorce or custody action.

When necessary and appropriate, provision of court testimony and/or participation in legal proceedings and preparation of materials for legal proceedings.
